As of 5/20/2020 users who have access to both the PPMC Portal and the new CORE Provider Portal will no longer be automatically redirected to the CORE Provider Portal upon login to the PPMC site. New and existing users must navigate directly to ca.coreportal.com using their existing login credentials (i.e user ID and password) to manage their assigned IPA membership. Users experiencing any issues with this process are advised to contact the CORE Provider Portal Support team via email at portalsupport@agilonhealth.com or give us a call at (877) 875-7329 toll-free, Monday-Friday, 9 a.m. to 5 p.m. EST.

Medicare Membership - Transition of Services

  • If you have an authorization that is already approved by PPMC prior to 1/1/19 and has not expired, there is no need to resubmit a new authorization.
  • For any status inquiries about authorizations submitted before 1/1/19 or for claims with a date of service prior to 1/1/19, please continue to use the legacy provider portal.

Medi-Cal & Commercial Membership - Transition of Services

  • If you have an authorization that is already approved by PPMC prior to 10/1/18 and has not expired, there is no need to resubmit a new authorization.
  • For any status inquiries about authorizations submitted before 10/1/18 or for claims with a date of service prior to 10/1/18, please continue to use the legacy provider portal.

As a provider, it remains your responsibility to verify continued eligibility PRIOR to administering services approved. If your approval has expired, you will need to submit a request via our new provider portal.
